요약Background: Despite significant fiscal and political investment, maternal mortality in the United States has more than doubled over the past 25 years, disproportionately impacting women of color. Integrated use of midwifery care in publicly funded healthcare institutions is a vital yet understudied element in narrowing this maternal health disparities gap. Midwives have a long history of caring for historically disenfranchised women and families, especially in New York City's publicly funded healthcare system. Their care model supports the physiological processes of childbearing, while centering on the complex web of personal, social, and cultural needs of a woman. This study offers insight into the personal, social, professional, and organizational elements that influence midwives' ability to build relationships with their clients and with their professional colleagues.Methods: Using a qualitative exploratory design, twenty full-scope midwives working the New York City public healthcare network were interviewed. Semi-structured interviews were designed to investigate relationships, organizational structures, challenges, and successes in actualizing midwifery care within the large healthcare system that serves historically disenfranchised communities. Using thematic analysis, guided by a reproductive justice framework, recurrent and interrelated themes, related to the central research questions, are identified and are described here.Findings: While midwives describe their care as relationally driven, they also describe the fundamental tension of operationalizing their practice model (termed Kairos care) within the dominant medical model of maternity care (termed Chronos system). One overarching theme 'Kairos care in a Chronos World' captures the inherent and persistent tension that exists in attempting to provide a relationship-based model of care (Kairos) in a system designed to value measurement, efficiency and pathology (Chronos).
