요약Deficits in stress-response systems are a hallmark characteristic of schizophrenia and psychosis spectrum illnesses, and recent evidence suggests that this impairment may be evident in adolescents at clinical high-risk (CHR) for the development of a psychotic disorder. The overarching aim of this novel dissertation was to evaluate different levels of biological and subjective stress reactivity as a combined biomarker to determine the overlapping and unique contributions of these measures to symptom severity and in predicting course of illness. The final sample included 57 CHR individuals and 66 healthy controls (HC). Participants underwent clinical assessment at baseline and at a one-year follow-up. Two domains of stress were measured from both a biological and subjective perspective: basal stress (stress in the absence of an acute stressor) and stress reactivity in response to an acute lab psychosocial stressor. Biological measures included basal and reactive salivary cortisol and reactive salivary alpha amylase, and subjective measures included self-report measures of daily stress and response to the acute lab-based stressor. Expectations of abnormal stress systems in the CHR group were generally supported. CHR participants had higher morning basal cortisol levels, a greater frequency in daily stressful events, higher levels of daily stress, and higher levels of stress prior to the lab stressor. Interestingly, CHR participants exhibited a blunted subjective stress response, and a trend suggested a blunted alpha amylase stress response to the acute stressor. A trend also emerged to suggest that basal stress predicted higher current positive symptoms
